Cycling is an excellent way to improve cardiovascular fitness, build lower-body strength and spend time outdoors. It can also support mental wellbeing, whether you enjoy a relaxed weekend ride, commute by bike, train for a race, or tackle technical trails.
Like any sport, however, cycling carries a risk of injury. Some injuries happen suddenly in a fall or collision, while others develop gradually through repeated movement. Increasing your mileage too quickly, skipping preparation and recovery, or riding a bike that’s not correctly set up or adjusted for your body can all contribute. Understanding the warning signs can help you act early and keep cycling safely.
Cyclists can experience both traumatic injuries and overuse problems. The type of riding you do, your position on the bike, your training load, and the terrain can all influence which parts of the body are placed under the most strain.
The clavicle, or collarbone, is the long bone that connects the breastbone to the shoulder blade. A fracture is a break in this bone and is usually caused by a traumatic crash.
How it happens: Landing directly on your shoulder or putting out an arm to break a fall can transfer force to the collarbone. These injuries are particularly associated with mountain biking, where uneven ground, obstacles, and higher-impact falls increase the risk, although they can happen to any cyclist.
Common symptoms of a clavical fracture:
immediate pain over the collarbone
swelling
bruising
difficulty lifting or moving the arm
the shoulder may sit lower than usual
a visible bump or change in the shape of the collarbone
A crash followed by severe pain, obvious deformity, numbness, or breathing difficulty needs urgent medical assessment.
Cyclists can injure the bones, joints, ligaments, or tendons of the foot and ankle. These injuries may happen suddenly in a fall or develop gradually through repeated strain.
How it happens: Clipless pedals secure cycling shoes with cleats. If a cleat doesn’t release when slowing or stopping, you may topple sideways with your foot still attached, twisting the ankle or striking it on the ground. Poor cleat positioning can also place unwanted stress through the foot and ankle over time.
Common symptoms of foot and ankle injuries:
pain
swelling
bruising
tenderness
stiffness
difficulty putting weight through the foot or ankle
Some cyclists notice discomfort only when clipping in, pedalling or walking after a ride.
Upper- and lower-back pain is common when the muscles and joints are overloaded by a sustained riding position.
How it happens: Cycling holds the body in a relatively fixed, forward-leaning position. Handlebars that are too low or far away, the wrong frame size, or an unsuitable saddle position can increase strain. Limited mobility and insufficient core strength or endurance can also make it harder to hold a comfortable posture as fatigue develops.
Common symptoms of back pain:
a dull ache
tightness or stiffness, particularly during longer rides or afterwards (lower back pain)
pain between the shoulder blades or around the neck and shoulders (upper back pain)
Pain that spreads into an arm or leg, or is accompanied by pins and needles or weakness, should be medically assessed.
Carpal tunnel syndrome occurs when the median nerve is compressed as it passes through a narrow space in the wrist. Symptoms may appear during a ride, afterwards, or at night.
How it happens: Sustained pressure through the hands, an extended wrist position, and vibration from the road or trail can irritate the nerve. The risk may be greater when riding position, fatigue or bike setup places too much body weight on the handlebars.
Common symptoms of carpal tunnel syndrome:
tingling
pins and needles
numbness or burning in the thumb, index finger, middle finger, and part of the ring finger
aching in the wrist or forearm
a weaker grip
difficulty with fine hand movements
The knee bends and straightens thousands of times during a ride. Small problems with bike fit or training load can therefore become significant, causing knee pain during riding and afterwards.
How anterior pain happens: Pain at the front of the knee is commonly associated with a saddle that is too low or too far forward, increasing knee flexion and loading around the kneecap.
How posterior pain happens: Pain behind the knee may be linked to a saddle that’s too high or too far back, causing the leg to overreach at the bottom of each pedal stroke. Heavy gears, a sudden rise in mileage, and cleat alignment may also contribute.
Common symptoms of anterior knee pain:
pain around or behind the kneecap
aching
tenderness
clicking
swelling
Common symptoms of posterior knee pain:
tightness, pulling, or discomfort at the back of the joint that worsens while pedalling
You should seek assessment by a specialist if you experience persistent pain, significant swelling, locking, or instability in your knee.

Not every injury can be prevented, particularly when another road user or an unexpected obstacle is involved. However, sensible preparation, good technique, and the right setup can reduce many avoidable risks.
ensure correct bike setup – check frame size, saddle height, fore-aft position, handlebar reach, and cleat alignment. A professional bike fit can be particularly useful if you ride frequently, have persistent discomfort, or have changed bikes
build up gradually – increase distance, climbing, and intensity in manageable steps. Sudden jumps in training load give muscles, tendons, and joints less time to adapt
warm up before harder efforts – begin with easy pedalling and gradually raise the pace before sprints, steep climbs, or demanding trail sections
strengthen and mobilise – regular work on core endurance, glutes, hips, legs, upper back, and shoulder stability can help you maintain control and position. Mobility work may also make a comfortable riding posture easier to sustain
practise using cleats – learn to unclip on both sides in a quiet, safe area. Release one foot early when approaching junctions or stops, and check that the mechanism is clean, maintained and set to an appropriate tension
use appropriate protective equipment – wear a correctly fitted helmet and visible, weather-appropriate clothing. Mountain bikers may benefit from gloves, eye protection, and additional protection suited to the terrain and style of riding
recover and respond to symptoms – allow rest between demanding rides, eat and drink appropriately, and prioritise sleep. Don’t ride through pain, numbness or weakness; early assessment can help prevent a minor issue becoming persistent

Following a crash, seek urgent help for severe pain, obvious deformity, loss of sensation, breathing difficulty, or an inability to bear weight.
